We offer a range of personal care and social support service ranging from 30mins to 24hours within the confines of your own home or in the community. The services we offer include:
Support to be appropriately clothed
Assistance to dress and undress.
Support to be manage personal hygiene and toileting
Assistance to have a wash or shower and management of continence care to maintain dignity.
Support to maintain nutrition
Assistance or prompting to ensure adequate food and drinks intake; prepare meals and support with food and groceries shopping.
Support to maintain health and management of medication
Assistance or prompting to take medication and/or administration of medication and liaising with health services for GP/hospital appointment and collections/ordering of medication.
Support to ensure safety at home and maintaining a habitable environment
Assistance to maintain essential amenities and complete household chores like pay bills, banking errands, cleaning, laundry, washing, vacuuming, tidying.
Support to develop and maintain family and personal relationships and social ties
Assistance to access the community to engage in activities of choice and to maintain personal family/friend and social ties, attending church and other social activities in the community.
Support for Carers
Assistance with sitting service or replacement carer for Personal Assistance or 24hrs support at home in an emergency or a crisis to prevent carers breakdown.
Support with mobility and transfers
Assistance to safely transferrin and out of bed/chair/toilet etc and ensure safe walks with mobility aids and equipments in line with appropriate moving and handling plan.
Support with social interaction and social inclusion
Assistance to link with wider community resources that will make a difference in our client’s life especially for those who struggle to go out to engage in meaningful activities of their choice or meet new people. We also go the extra mile to seek and provide information and signposting our clients to other relevant services to meet their needs in a holistic manner.
